You would think by now, brakes used to slow down spinning shafts would be fail safe.

Did he died?

From Shelby County Today:

At approximately 3 PM, Tuesday, July 2, a 2008 Dodge Ram 2500, pulling a new travel trailer, hit the corner of the Texas State Bank Drive-In on Tenaha Street, causing the complete awning to come to the ground. Luckily, the driver of the Dodge, Mickey Miller, of Garrison and his 10 year old son were able to exit their vehicle uninjured.N

According to Miller he was circling the bank to park on the other side when the corner of his travel trailer caught the corner of the bank awning. Next thing he heard was a rumble as the awning started falling against his driver's side door. Somehow he was able to unbuckle the seat beat and exit the other side of his truck as it was falling.N

No one died.  Just some doink who doesn't know how to drive his RV.  Put it back up!

I used to work at a place that had an arch way at the beginning of the driveway that was fairly low.  About 9' or so.  We kept adding MORE and bigger signs warning drivers of the height and to use our other driveway that didn't have the arch.  It got hit by tour buses and RVs at least twice a month.  I witnessed a few ruined AC units and other damage.

I made an "IN" gif when I first saw that months ago.  I'll need to re-post it as soon as I get home.

I saw this a while back. I think I remember a report saying he made it out alive. I want to say kids were in there too and all unhurt, if I remember right.

How did that happen?

Looks like something exploded at the midsection.

Wind turbine overspeed during high winds in a storm, which ultimately led to catastrophic blade failure (the tip speed of those blades during normal speed is already quite fast).  Normally, wind turbines are shutdown once wind speed reaches a certain threshold to ensure things like this don't happen.   IIRC, this one's rotor brake failed and allowed the overspeed to occur.  I believe they knew the brake had failed, and that's why the camera was pointed in its direction.

ETA:  The below is obviously not directed at g3shooter.

Brakes aren't the main stopping source in a turbine, the blades are, but the above assessment is correct I think.

I may be incorrect here, but aren't the generator exciters used to lock up the rotors?  Mechanical brakes are secondary, correct?
