I think Surefire is a company that simply doesn't want to tolerate anyone running their name through the mud. It has more to do with a company reputation than silencers IMO.
The KAC M4QD is loud because of a requirement for it to run 210 rounds in 3 minutes. The bullets begin to be marginally stable around 120 and the bore needs to be ~.290" to pass the marginally stable bullets. The large bore creates a situation where it is harder to make the silencer efficiently reduce the sound.
Loud cans will also have more flash than quieter ones most of the time. The surefire silencers may be an exception as they have an internal patent pending device that is supposed to reduce flash.
Different parts of the military are looking for different things for different applications, the M4QD was a general idiot proof silencer for the troops. The Ops 12th model was procured as a signature reduction device for skilled operators using precision weapons, similar to the application for the silencer on the 110 SASS.
