So I purchased a CNC Warrior galil receiver a while back and sent my galil kit to be built as rifle.

Since I intend to SBR it  I have a question about what needs to go into question 4a adn 4d.  The form states Name and Address of the original manufacturer.

On just the receiver it is engraved with the following:

CNCWARRIOR USA
Charlotte, NC

This is exactly how it is on the receiver

Now on the model the receiver is engraved with
Model GL-A01

Should I just skid the address and just list what is EXACTLY on the receiver?
Should I spell out what is EXACTLY on the receiver or just "GL-A01" with the dash?

I missed the "R" im sick and dont feel good.  AS far the location the website says something diffrent.
