Is it needed?

Going to have a barrel reprofiled for a gas block I am having made. The old shoulder will have to be cut down, do I need to cut a new one?

Not if it's secured properly with taper pins or dimples for the setscrews.  The shoulder does two things:  secures the handguard endcap, and helps locate the gas block/sight base properly over the gas port the very first time it's installed.
