FYI, Ratwork does not make the extractor claws in house, that is a standard USGI spec product installed in the bolt head.
Even high quality extractors can fail, I had a Colt extractor break during a Kyle Defoor class and just had to muscle through the CQB stage with my gun as a pull action rifle.
If you have broken two bolt sleeves, had an extractor fail, and with the wear pattern on the bolt face I am wondering if you may have an issue with the MSAR receiver and/or barrel itself. You shouldn't really be seeing wear on the face of the bolt like that, at least not in my experience- the bolt should not be contacting the barrel breech, but the wear marks suggest that is happening, or that it is rubbing somewhere in the cycling process. That force on the face of the extractor (and also the bolt sleeve impacting the receiver) could theoretically be causing the failures you are seeing.
I am not saying that is 100% the issue, but just pointing out something I noticed that seems a bit off.
Might be worth some more investigation.
