If so, you don't fill out anything on the back. Read all of the instructions on the form.

Correct. I've said it twice already.

You have to print pages 1&2 front/back, but if filing as an entity, and the item being transferred is a suppressor or AOW, page 2 doesn't need to be filled in. (If you were transferring an SBR/SBS/MG/DD, you'd only fill out section 14).

Read the instructions, specifically 2e and 2f.

Every one of my F4s (over a dozen cans and an AOW) are completely blank on the back. I've never even seen one of them until the stamp is affixed. Dealer simply fills out the front and sends it in. (All my 20+ F1s are the same...blank page 2. If you use eForms, it doesn't even prompt you for those yes/no questions)

(Checking the boxes, or signing it, or filling out #14 won't cause a problem...it's just unnecessary)
