I've seen a few companies that mill the slide and at least one mount that fits in the rear sight dovetail. Which is the best option? Any recommendations for manufacturers or companies that do it?

The highest performance option will always be a custom milled slot cut for your individual optic. ATEi and L&M are high quality, well known options. Fire4Effect is a company I have been looking into that appears to perform high quality work, and might be more flexible/available considering their size.
