What would cause this? Its a 5.56 I believe. It might be a .223, not sure what I was shooting at that particular moment.

Bullet hitting a lug on the barrel extension instead of the feedramp.

The old casing broke in half. The back half got ejected and the new bullet got wedged into the old casing.

The case in the chamber before last blew in half, leaving the front half lodged in the chamber. The next round to slam in there
locked into the broken case, and extracted the broken case out with it. This was taught as an emergency broken case extraction
method back in the day. Although this one was obviously not done on purpose.

What they said - I'm on a phone and didn't notice the extra brass, just saw the projectile pushed back in the pic.

Clean your chamber with a fresh/new chamber brush.  A worn one is worthless.

Its less than two years old. I am not sure what happened but I think its an ammo issue, not a gun issue.

I was wondering if it was a serious issue or not haha just wanted to be safe.

Don't feel bad, I was about to say what you said when I caught the extra case neck/shoulder.

Correct............an ammo issue not rifle.............you had a case separation.
Yours was higher on the case than I've seen but that is what you had.

Check your other brass, unless they show stretch marks or something, its most likely an ammo problem.  

Are these reloads?

OP multiple causes but old/poorly resized brass or bad headspace can be a problem.
