16",Mil-Spec 11595E - Barrel Steel (CMV)  , Chrome Lined, Carbine Length Gas System, M4 Feedramps, A2 FSB, 1X7 Twist, Muzzle Threaded 1/2X28TPI, 556 Chamber, HPT/MPI Tested, Gov't Profile
From a reputable Company like FN, Colt(DD and BCM do not make this)
Here's the problem= NO LtWt or M4/M203 Cutout Barrel Profiles

Heres a FNH barrel, that meets all your requirements except it does not have a front sight base.
