Steel WPA.

Shooting a HK23Ek, heard a pop vs a bang, belt stopped, pulled the barrel and found this






the primer smashed the round into the chamber, got it stuck and then ripped the rim off.

2nd time its happened in thousands of rounds with 223, had some 308 that tried to kaboom my hk21-wolf replaced it.

put the barrel back and finished the belt.

Years ago, (like I paid ~$100 for a case) I had 3 or 4 rounds like that in a case(1k) of amerc .223.

I've seen a couple in WWB 9mm, probably both from the same lot.  Primers backed out part way and locked the pistols up good.  I snagged one of the rounds, still have it laying around somewhere.  There was no damage to the rim and the area where the flash hole should've been was completely flat, no indentation like the ones here.
