Definitely suppressor exposed.
I don't know what kind of can or mounting system you have, but usually, the can would live on the rifle if you were going to run it under the handguard. That, or some kind of flash can.
Being that cans are expensive, I have .308 cans that I swap around between x39, 5.56 and .308, so having the rail behind the can is good for me. Plus, you can get thinner and lighter rails going this route :P.
For something like my 4.5" .22, where all rounds are subsonic, leaving the can under a handguard is fine since I have no interest in shooting it unsuppressed.
