Oral thermometers have a longer Mercury ampule/bulb (the silver thing at the bottom of the thermometer).
Rectal thermometers have a shorter Mercury ampule/bulb. |

this past spring we had a couple nasty bugs go through the house...... it hit the wife and i first....then the kids. after i had already used the thermometer on myself multiple times (orally) i saw my wife use the same one on the kids - under their arms... 
"yeah, i always do that!"
