I have put the drive in multiple USB ports on multiple PCs both Windows XP and 7.
All the computers will see the drive as a generic drive E, but none can access or do anything to it.
If you try to format or read or anything its either drive not found, you don't have access, or insert disk.

Anyway to fix this?

It's an 16gig Kingston DT101 G2

Some drives have a tiny drive lock sliding switch to prevent overwriting so look for something like that.  It's possible that it was formatted with a Linux or Apple machine and it's readable only by them.  Also possibly protected with TrueCrypt or something like that.  Is there a light on the USB that comes on or flickers?  Just might be DOA....

You probably need to repartition the drive and then reformat it.

Try running a very solid recovery utility on it.

XP and 7 have an awful lot in common...

It's a long shot, but try your flash drive on a machine that's running an entirely different operating system - Something like Linux or MacOS.
