Some drives have a tiny drive lock sliding switch to prevent overwriting so look for something like that. It's possible that it was formatted with a Linux or Apple machine and it's readable only by them. Also possibly protected with TrueCrypt or something like that. Is there a light on the USB that comes on or flickers? Just might be DOA....

You probably need to repartition the drive and then reformat it.

Try running a very solid recovery utility on it.
