You seem to know a lot without knowing the circumstances of what happened to that piece of brass.

The brass tells the story its self. Not sure if the light in the video gives you enough detail to see the stains from the carbon and it really shows up the area that was stretched in the chamber.  

I never could get a video to post properly on this board but I thought it interesting enough to post.

I know somebody that accidentally got a .40 mixed in with his 45 ammo, loaded the .40 in a 1911 magazine(yes, the feed lips will hold it in) and tried to shoot it.  The .40 went into the chamber, but the extractor didn't get a hold of it.  When it went click he reached up and racked the slide which chambered a 45 right behind the 40.  Boom!

Believe it or not, we were able to save the frame and slide.  I had to remove the barrel from the slide with a 1/2" ball end mill though.

most of the time it looks like an unfired round with either a really light primer strike or a strike on the rim... but from time to time someone gets lucky and sets one off (more often then not they go off out of battery)... every once in a blue moon someone gets on that will go off in the chamber and it comes out looking about the same as a 9 in a 40...

most of the time they come out something like these... the ones that go off out of battery are usuially split

had a guy i was shooting with do just that....
put a 223 in my armalite after he emptied the loaded mag i handed him....
figured it was cool, because "all ar's shoot the same stuff"  
i was trying to sight in another buddies ar with his new eotech...
took the rifle away from him....later on, same guy almost blew the toes off his right foot with a 12 ga...
he was in the middle of asking "how far you gotta push the thing forward before it'll go *boom*"

haven't gone shooting with him since.

Scariest piece of brass I have found was a .44 mag blown out as it had been shot in a .45 LC...a original, first gen SAA. according to the range staff he made it thought a full cylinder and was getting ready to load it again when someone stopped him.
