Mostly cold. Some...such as some English Bitters and Cask ales, I prefer at "cellar" temperatures. (60 some degrees)

Depends on the beer. Most I drink are better at around 55-58 but most of my summer beers are cold friendly.

Free and cool, but not ice cold.
I like beers with a little more hops and they tend to not taste as good when ice cold IMO.
