Do I need a sworn affidavit for each form I fill out/for each assault weapon I need to register? Thanks

Yes for each AW you register  you need 1 of the following: a copyof a  DPS-3,a sales receipt or a signed affadavit

The interesting thing is the stupid form doesn't specifically say one affidavit for each application. Rather it says the following:

"Submit proof that you purchased this weapon on or prior to April 4, 2013 in the form of a valid sales receipt and/or a DPS-3, or sworn affidavit that the specified assault weapon was purchased in compliance with state and federal laws."

Technically if you submitted one sworn affidavit that listed all the specified "assault weapons" your applying for you have technically complied with the language of the application form. So far I haven't heard of anyone submitting a single sworn affidavit for multiple applications, both multiple certificate of possessions and the magazine declaration form. I'm hoping someone will, so we can see what happens.

For some who pay to have the sworn affidavit done, it could get rather expensive if they have multiple forms/affidavits to notarize.

I understand what you are saying. But I would not dicker with legalise with these people.THe safe money is to do a affadavit for each one and send it with the paperwork for each AW registration.

No one should be paying to get a sworn affadavit done. Write something up on a word document and take it to your bank to get notarized(which they will do for free for their customers)

Not everyone knows that their bank may do them for free. Previously, in one of the threads either here or elsewhere, we had one or two people who were complaining about the cost to get them done because they didn't check with the bank they belong to (or their employer who may have a notary on staff).

Interesting, have a few questions that may help others when it comes to choosing to register.

Did you mail it in? Or did you dropped it off at Middletown?
If dropped off at Middletown, did they say anything about using one affidavit for multiple certificate of possession applications?
Did you also include your magazines that were being declared on the same affidavit? I.e. Submit a magazine declaration form and use the same sworn affidavit as the certificate of possession forms.
Have you received anything back from the state indicting your applications were approved? I.e. The bottom half of the certificate of possession back from the state.

Thanks for the clarification.  Just went to the local townhall and they were great sports about notarizing all the forms and affidavits. One women was like "my husband needs to get on top of his registration!" I do not plan on sending mine in for at least another couple of months. I just wanted to get all the work on my end squared away.
