Its my first time attempting a form 1 SBR (for an SLR104ur) and a bit lost and confused with the process and documentation.

On the RPQ, item 4b. Name & Address of Maker, Manufacturer and/or Importer of Firearm: Would I input my Trust name and address here since I am making an SBR or would I input Arsenal, Inc. and address since they imported the firearm?

On item 3e. Photograph: This is a digital form --- how do I upload my photo here if there is no upload option? Snap shot and paste the image over this space?

Since the gun has already been made by someone else and has preexisting markings like a model and serial number, you put in the name of the company that made or imported it.  You’re taking a preexisting gun and making it into a SBR.  You only use your name or trust name if it’s something like an 80% lower that you completed.
