I just saw this thread and have some training from Paul Castle.

Keep in mind its a pretty controversial method and instructor. For me it has been a good tool in the tool box, but it's not for everything or everyone. I kept an open mind and use some of it and some of the material I tossed.

The High position is shooting from midchest height and just below the eyeline, This is a position for basic close range, i.e. touching. Extended is the sighted shooting position. It also is designed for close encounters, but more so out to 10-15 yards if my memory serves me right. Anything past that he said is for conventional shooting positions.

These are for very close range or awkward positions, such as shooting from a vehicle or in tight quarters. The Extended position uses the opposite eye for sighting. This will be confusing to some, and he gave some reasons why the natural points of such and such eye relief, but once it is taught felt pretty natural. My weak hand shooting improved after trying these techinques and I use some of it for very close quarters longgun manipulation.

I have an LE friend who took the training who agrees it is very good for tight quarters such as blind spots on vehicles.
