Just to make sure, that's two form 4's with orginal inked signatures on both? In other words you can't fill one out sign it and make a copy of that one. |

Are you using the most current Form 4 ?
Did you put down the trust as the transferee (and not your own name) ? Did you generate both pages of the Form 4 on one sheet of paper (front /back) instead of two ? Send two copies of the Form 4. Copy of trust w/ property sheet (schedule something or other). Cert of Compliance form (though it may not be needed with a trust). Check made payable to Dept of Justice ? |

Check made payable to Dept of Justice ? Paper said "Bureau of ATF" You need to send a copy of the Schedule A, and the Schedule A needs to have some type of property listed in it. If you don't, ATF will send you a problem letter. |
