Archived [ARCHIVED THREAD] - So, what goes in box 6a and 8a of a form 4?

My friend just filled out the form 4 at a local shop today and they want his SSN there. That box is for the SOT license, right? If he has no SOT licnse, leave it blank. Mine was left blank but mine is still pending.

Item 6a should be left blank, unless the transferee (buyer) is an SOT.  Similarly, item 8a should be left blank, unless the transferor (seller) is an SOT.

I thought so. I also confirmed it with my dealer as well.
