I've searched the ATF site but I came up with a goose egg.  Any pointers?

They probably want you to ask for the form like they do now on the original C&R application.  Too simple having stuff online that you can download.<G>

This is the one I used when I moved.  I had to copy my lease and attach it.

Surplusrifle.com also has a page with a little info on moving, it is at the bottom of the page:

Thanks. You are correct on that form.  While digging through my FFL paperwork, I found where the ATF specifially says that is the right form.
