I am picking up an M1 is a little while , whats the process for getting a curio and relic form so I can have it shipped to me (price of form, process of obtaining, times it takes to obtain, etc )

Form is free to download, Cruffler's has a link to it.  Cost of the license is $30 for 3 years.  You will send one copy to your local chief law enforcement officer, and one to the ATF.  Takes from 4-8 weeks.

fwiw, it took 8 weeks from the time I put the app in the mail to getting the license in hand.  lots of great info will be sent also.
