I recently shot in the Southern Regional IDPA Championship in Manchester, TN.  One the very first stage I slipped in the wet grass and went down hard.  After the shock of wiping out wore off; I noticed that my ankle hurt...bad.  Luckily for me, there wasn't a lot of footwork involved in that match and I pulled out a 3rd place finish in my division.

Had eotech batts die

Had rear buis fall off. forgot to locktite it

Started a stage without hearing stuff, fired one round and had to stop and get hearing protection. (I had ear plugs, but always double up)

The worse mishap I have personally experienced was cutting my thumb open on a folding knife that opened in my pocket while I was taping targets.  At first everyone thought I had caught a vicious ricochet off a steel target.  

Second worst was shooting from a low port and catching my front sight on the top edge of the port, sending it flying, never to be seen again (good thing it was my last stage of the day).

I have seen a few shooters go ass-over-tea kettle in wet conditions when stepping on pressure panels that activated moving targets.  At least one lost control of a loaded firearm and was DQed.  Those things need some grip tape applied to them!

Area 4 last year. It rained a couple of inches the night before and we had muddy conditions all day. Towards the end of the day it started to dry up a bit and not be as slick, so I decided to really move on the next stage I shot. Well, it turns out your feet don't move as fast when there are a couple pounds of mud caked on your shoes
. I came about an inch from crashing my head into a table. I kept my finger off the trigger, didn't let the gun hit the ground, and got up and finished the stage.

Did a 3 gun match before I knew that some AR magazines are crap.  I had half of them double feed on one stage.  I got real good at clearing the jams.  Got a few compliments on how quickly I was able to recover, but it still sucked to waste so much time.

Not a match mishap per se, but did manage to have a heart attack about six hours after a three gun match.

Why? I understand that repeated exposure is a really bad idea, but I hunt without hearing protection and if I'm ever in a self defense shooting I won't have hearing protection...?

I wouldn't shoot a whole stage with NO hearing protection.  But he said he had plugs in and stopped shooting cause he didn't have muffs on too.  That's a little especially if he didn't get to reshoot the stage.

Laid into the first 3 targets with serious round count so I could reload before I got to the wall, both SO had terrified looks on there faces after I showed clear & holstered.
After I explained myself they patted me on the head & said,"good,at least your thinking, DON'T DO IT AGAIN!"

Shot an entire stage with no hearing protection.  After the initial shock of the first shot, and the slight delay, it was no problem.

Had my rear sight fly completely off in the middle of a stage, finished the stage, but not very accurately.

Had a shooter this past weekend on the 1st stage not being able to get his 9mm Beretta into battery.  Turns out he didn't realize the 9mm ammo he bought was 9mm Markov.  I'm just thankfull the gun didn't go bang!

During a reload I had slam fire.  I looked up & waited for the RO's hand to touch my shoulder & for him to say unload & show clear.  When he didn't say anything I pressed on w/ the stage.  Turned out I had three holes in one target.  A perfect 'Mozambique', 2 to the body & 1 to the head.  RO said my muzzle was down range & could not tell if the shot was intentional or not.  Since it was a 'safe' shot, and no miss placed holes, he let me press on.

Way back when I first started to shoot I used a Ruger P85.  Ramline just came out w/ a 18 round magazine for it.  My mentor was a big time gamer and for this one stage he suggested a shooter could run to the center of the charge line, go down on one knee & from that position the shooter could shoot all 9 targets.  Needless to say I took his suggestion & ran with it.  I blazed all 9 targets w/ 2 each & looked up to find no RO standing next to me.  He was startled by 'my technique' & was not expecting me to run but to walk & shoot as I saw them.
