my app was apparently incomplete because i forgot to sign the phsyciatic treatment page, i guess i just glanced over it since I have never recieved any, its got the blank space and the "make corrections if needed" space, I assume i would just leave the whole thing blank and sign at the bottom, is this correct? thanks in advance
