I believe this happened to a couple other departments about 2 years ago and at that time they pulled ALL of their G-21s off the street and went to Sig P220s... LA County was one department as I recall and another on the East side... Nothing to see here.

Granted we don't know what ammunition was being used. Lots of departments use commercial reloads for training, And it has been my experience that most Commercial reloaders SUCK.... Just saying.

OUCH!!  Hope the guy is alright.

The ammo mfr will deny is was their product's fault.  Glock will replace the pistol at no charge, with few questions.  Hopefully the Deputy(?) will be alright.  Hopefully neither of my G21s ever try that shit on me!

They said it was the first time the pistol has ever been "used".  If that was truely the first shot fired, I would rule out ammo due to the odds.  Perhaps they had an excessive amount of oil in the barrel?  I've had some guns that were literally dripping with oil when I got them––Walther TPH and the Taurus 709––but never with the six new Glocks I've opened over the years.

I am not a big Glock fan. Having said that, I have a feeling that the .45acp ammunition used were either reloads, solid lead bullets (no jackets), or both.

The interview shows poor trigger finger control by Chief Deputy Kevin Cecil, yes the trigger is gone but he comes very close to sweeping the reporter with his finger inside the trigger guard.

RULE I: ALL GUNS ARE ALWAYS LOADED
RULE II: NEVER LET THE MUZZLE COVER ANYTHING YOU ARE NOT WILLING TO DESTROY
RULE III: KEEP YOUR FINGER OFF THE TRIGGER UNTIL YOUR SIGHTS ARE ON THE TARGET
RULE IV: BE SURE OF YOUR TARGET

On this one, I might cut him some slack.  But yes, as a general rule I try to handle all guns the same way all the time, regardless of whether or not it is in functional condition.
