So decided I wanted to SBR one of my lowers and went online to order paperwork from the ATF. Ordered 3 packets of Form 1,4 and the declare I am a citizen form along with a C&R book listing all the weapons that can be purchased. Saturday 2 envelopes showed up with everything I ordered except the Form 1 and 4 papers. A separate letter said they are back ordered on the Form 1 and 4 because the sudden increase in submissions  I know should have done this a long time ago but life always gets in the way

As stated, you can download ATF Form 5320.1 (May 2016 Rev), printed front & back.
You can also order the fingerprint cards (FBI FD-258), direct from the FBI, for free. Just put the following in the "ORI Number" box: WVATF0900 ATF–NATL EXPL LIC MARTINSBURG WV (ATF does this for you when you get the cards from them) |

Before he prints you, you need to specify you need blue FBI cards. |

The ones that they sent me were WVATF0800, which is the National Trace Center ORI. |

I've also heard that they cross your ORI number out and write some arbitrary number in its place. |

You can fill in all the stuff that doesn't change like your info and the "yes" "no" boxes then print like 10 sets. When you want to F1 something just hand write in the info and send off. |
