First let me say sorry cause I'm sure this gets beaten to death here but.....
I'm filling out my SBR form to send to my local PD so...A few questions on form 1...

4a - name and location of original manufacturer, where the lower was originally built but its an eagle arms do I put what is on the lower or armalites info?

4b - type of firearm to be made, Short Barreled Rifle - this probably seems obvious but i really do no want to screw this up.

4c - calliber / gauge, I am going to complete a 5.56 upper to start but im thinking about adding a .300 blackout can i put both?

4e - barrell legnth, I'm thinking im going to do an 11.5 but can I put 10.5-12.5 just in case i build multiple?

4f - anyone know the approximent length of the rifle with an 11.5" barrel and carbine buffertube (i'm pretty sure it goes by fully extended not collapsed correct?)

4h - additional description - none

4i - why you intend to make this firearm, sporting/personal collection

6a & b - ocupational tax status - employer ID number & class (would this be on my taxes?)

anything else you guys think will help?

4a - name and location of original manufacturer, where the lower was originally built but its an eagle arms do I put what is on the lower or armalites info?
Put down whatever was originally stamped on the firearm by the manufacturer

4b - type of firearm to be made, Short Barreled Rifle - this probably seems obvious but i really do no want to screw this up.
Correct

4c - calliber / gauge, I am going to complete a 5.56 upper to start but im thinking about adding a .300 blackout can i put both?
You may only list one caliber on the Form 1

4e - barrell legnth, I'm thinking im going to do an 11.5 but can I put 10.5-12.5 just in case i build multiple?
You may only list one barrel length on the Form 1

4f - anyone know the approximent length of the rifle with an 11.5" barrel and carbine buffertube (i'm pretty sure it goes by fully extended not collapsed correct?)
Fully extend the stock and the overall length will be approximately 30"

4h - additional description - none
Correct, leave this blank

4i - why you intend to make this firearm, sporting/personal collection
"for any lawful purpose, including personal collection and sporting use"

6a & b - ocupational tax status - employer ID number & class (would this be on my taxes?)
Leave blank, that is for licensed manufacturers

anything else you guys think will help?

List whatever barrel length and caliber you plan on building first.  You can always swap uppers later but ATF will have a fit if you try and register it as more than one caliber and barrel length.

PRINT IT OUT DOUBLE-SIDED, 2 COPIES TOTAL

Good luck

1 more question for you....what barrel length? I'm between 10.5 and 11.5, the 12.5+ muzzle break just brings it too close to 16" for me to justify paying for the SBR
Also where do I get the FBI fingerprint cards? Does MPD have them?

I've always ordered my Form 1's and fingerprints cards from the ATF.  You can do that here:  http://www.ar15.com/forums/t_2_138/174764__Q__Why_BCM_chose_the_11_5_SBR_over_the_10_5_.html

Regarding barrel, there are a number of factors to consider:

1.  Will you be running a suppressor?
2.  Do you have a specific barrel manufacturer in mind?
3.  What size and type of muzzle device will you be running?
4.  Will this be just a range toy or would the improved ballistics of longer barrels matter to you?

Good read on 10.5 vs. 11.5 here:

Why BCM chose the 11.5 over the 10.5

possibly very far down the road

not yet any suggestions?

flash suppressor not sure on the size or make yet

I did find this article somewhere and though it was very interesting and thats what made me lean towards the 11.5 vs the 10.5 but I was wondering about personal experinces between the two, if anyone has any input?

I run/have run 10.5's, 11.5's, and 12.5's.  All function(ed) without issue.  I'm not sure that a general monologue on my experiences will be helpful as each gun serves a particular role for me, but I'm happy to answer any specific questions you might have.  Fire away...

Can you see a diference when shoot at 50 yards with a 10.5" vs 11.5" barrel? What barrel do you recommend? what is the best twist rate for a barrel of those lengths? 1-7 or 1-8? shotting mainly 5.56 55gr.

Best buffer weight H or H2?

If you care about clone rifles go with the 10.X, if you dont, go with the 11.5".
But to answer your questions-no no difference even to further distances.
10.3"-DD, 11.5" -BCM-My personal opinion.
Stick with 1-7 for the shorter barrels.
Start with a standard buffer, and then jump to H2 if your having issues.

Listen to Andrapos, he walked me through my first Form 1 with success

ETA:  Like Andrapos said; Only put one firearm(length, caliber, etc.) on the form.  Later on you can do whatever you want.  You are SBRing a lower.  Once its SBR'd it doesnt matter what you put on top of it(caliber, length, etc.)

I'll echo a lot of what b-rod said.

You won't "see" a difference btwn 10.5/11.5 unless you start pushing the distance (shorter barrel = less velocity = more bullet drop) or start hitting bad guys because M193 ammo needs velocity to work.

I'd also recommend the faster twist barrels as you may at some point want to run heavier duty/SD ammo.

If properly gassed, you should be able to run your SBR with an H or H2 buffer, but it really depends on what ammo you are running (hot mil ammo or something under powered like Tula) and whether you have an in spec gas port or one that has been opened up to accommodate the kind of crappy ammo that many shoot.  Point being, you'll have to experiment once the gun is up and running.

I prefer BCM and Noveske barrels (DD is fine too), but you should do some reading on how certain companies size their gas ports.

Shoot me a PM if you ever make it to Highrock and want to test drive different barrel length SBRs.

You will need to request the FD-258LE finger print cards online as most local PD's dont have them. I may have 4 spares I can mail you or drop off if you are local.

Good luck man!  The last time I went to get my fingerprints done I had my FBI cards with me, and the officer doing them had never seen these cards before....and didnt understand why he couldnt just do them digitally because the ink rolling method is messy I kid you not
Your fingerprints HAVE to be done on the BLUE FBI cards, so don't let them tell you not to worry, and what they have is fine-if they aren't the correct cards, don't bother doing them.

I brought my cards to Middletown Department of Confliting Interpretations (DESPP) they fed them into the digital printer. Worked out great.
