Do each LG AW Registration form have to be Notarized ? It has A Notary signature section on it.Thanks.

Yes.

Note the certificate of possession for assault weapon form is for both rifles, shotguns and handguns that are classified as assault weapons.

I thought if you are submitting a sales rec't or DPS-3 you didn't need a notary?  I figured if you were using a sworn affidavit then you would need a notary stamp.

the form itself requires a notary, no matter what your proof of legal possession is.

What information on the form would give you that idea? The application form has a section for the notary to sign, their notary expiration, and affix their notary stamp. The sworn affidavit is a separate legal document you can include if you don't want to include the DPS-3-C or sales receipts with the application.
