im an NRA certified pistol instructor and need a "massachusetts basic firearms safety course instructor application" cant find a pdf anywhere mass.gov only has one for LEO any help would be great ....thanks

They aren't online.
Contact the MA State Police Firearms Division at HQ (Framingham).
I don't have a phone number, but Google will probably get you that info. They should mail you out a packet of what you need.
