assuming you're on AT&T, don't get issued a new SIM card? I believe i did with my last phone. But if you bought a phone to use with your AT&T service, it should just swap right in. I've done that recently.

Phones are usually assigned a carrier. Only that carrier's sim cards will work.

The solution is to buy an unlocked phone, which works for any carrier or find someone that has the unlock code for your phone.

I called At&T and told them my phone kept locking up and asked for the unlock code. They gave it to me

if it's an unlocked GSM phone, then you can swap SIMs. at&t (cingular) and t mobile are the 2 major gsm carriers in vegas.
depends on the phone, you can unlock it yourself or ask for the unlock code (subsidy unlock code) from the phone's carrier. you can also purchase the unlock code online.
