I have done it for a supressor.  You might want to check on the Class III board or search the web for blank trust documents.  Make sure the language is vailid in PA however.

By the way I used Willmaker software to make my trust, got it notarized, etc.  Just sent it all off this week to the ATF for a SBR.  I will see how it goes.

Thank you all for the replies.  

Edit:  Also I guess I ought to say that I am an attorney and could use one of the "form" trust documents that we have in the office (in fact I have one on my desk right now).  

However, in July of 2006, Gov. Rendell executed the PA Uniform Trust Act (UTA) which creates certain reporting requirements to beneficiaries of certain trusts - amongst other things.  I do not, as of now, think that this applies to revocable trusts (which in one form or another is what we are using to get BATF approval) if the settlor is still living i.e. if it is your trust and you are still alive it would be kind of stupid to have to legally report to yourself something that you did with trust assets - but don't get me started on stupid legal requirements - we would be here for the next decade.  

However, I have yet to completely read the Act and then think about its possible consequences and for obvious reasons, I don't want to do something foolish.   It appears that the primary purpose of the UTA is to prevent trustees from running trust assets into the ground thus screwing the beneficiaries and potential beneficiaries of the trust.  But we shall see.

Let us know if you find anything in the PA Uniform Trust Act, that would effect revocable trusts.
