Unless things have changed, the post office has all the forms.

You'll need: A Certified COPY, (not the original), of your birth certificate and photos.

Once all s submitted....about 3-4 weeks unless expedited. As I remember.

Google should tell you everything.

Yes, post office.  You need certain sized and type photo also.  You may need to provide the photos necessary if the post office doesn't have picture ability.

The forms can be dowloaded from te gvmt website. Google "passport applocation".

Walgreens or Kinkos can do passport photos in addition to the post office.

There are several levels of expediting available depending on your need and budget.  I think standard is 5 weeks using US Mail and super expedited is a few days using companies that walk the paperwork through one of the passport issuing offices, and FedEx overnight to ship the documents.

get your passport pictures taken at Walgreens. They are the cheapest by far. $9.99 for two 2 x 2 pics.

Be careful at walgreens. Gov is super picky on the photos. I
went to the court and it went super fast no apt needed.

Just got one for me and my son last year. You'll need the ORIGINAL birth certificate. Yes, I was nervous about sending it off for both of us. Passport sized photos...2X2 I think. We got ours at Cosco, cheap, painless and quick. The Post Office is your place to start the process. They'll have all the application forms, instructions and fees. Once we sent the paperwork in it was about 2 months before they showed up at the door. And yes, the birth certificates came back.

-downloaded all files in pdf format and filled them out and printed them off
-acquired embossed official copy of birth cert from county I was born in in CA
-made appt with post office that had photo services
-showed up to PO
-PO checked over my paperwork
-PO took my photo
-Gave PO money

30 days to the day my passport arrived.  IIRC they sent my birth cert back separately.

I'd say the hardest part was getting some of the info they wanted on the paperwork, parental info.. not that hard at all really lol.
