I need a physician to sign off some documents for me.
The document is a physician's authorization for a physical agility test. This basically states that I can safely perform the test. Thanks. ETA* to add South FL. |
| This may not go over too well with some physicians. I work in a Dr.'s office and we are pretty picky about people walking in and asking us to sign release forms for them. Dr.'s licenses are on the line, and if something happens to you, well, you always have the right to sue, and that raises our malpractice insurance. Just something to think about. |

Just something to think about. sorry If I wasn't more explicit to what I am looking for, I'm not asking for anything illegitimate. whatever is necessary to be completed to get the sign off is what I'm looking for. I'd imagine that I would need a physical, evaluation etc... I don't expect to walk in to someone's office and have them sign something. I can go to Hialeah for that.
