[ARCHIVED THREAD] - CCW permit question

I am prior service Army so I can use my DD214 as proof of firearms proficiency. However, it also says in the instructions that a notarized affidavit must accompany proof of firearms proficiency. Do I need an affidavit for my DD214 or is it ok by itself? I don't even know how I would go about getting one if I did need one.

List of things to send:
Notarized application
Receipt for digital fingerprints
passport photo
$75 check
Copy of DD214

Anything I am missing?

I used my DD214 and nothing else, but that was 20 years ago, maybe things have changed.

Ok. It sounded like the affidavit was for the instructor to fill out. I'll give it a shot. Check goes in the mail tomorrow!

i just mailed a copy of a list when i did small arms training. worked just fine.

Offspring, I just got mine two years ago and just my DD214 was fine. I sent EXACTLY the same package you listed with NO problems.
