almost a decade after getting my 01 i'm ready to get off my ass and become an SOT payer

i'm filling out the 5630.7 though and have a question about #9 "tax period covering"

do i put the first date as the date of the application or the date of the first day in the tax period?

i.e. today or july 1, 2012?

also, #16

am i a new business?

The SOT tax year runs from July 1 through June 30, so you would enter 07/01/2012 - 06/30/2013. When you pay again this May you would enter 07/01/2013 - 06/30/2014.

If you've never paid the SOT then I think you select New.
