I have just over 1200 rounds through my SCAR16. In that time I have had two double feeds. Both were on the first use of the magazines in question and both were on the second round cycle. The first occurred with the FNH magazine that came with my SCAR on the first shoot. The second happened this week on the first use of a new Brownell magazine. I have eleven other Brownell magazines with no problems. I inspected both double feed magazines and observed a small burr that was rubbing and shiny on both magazines, located in the bottom left (looking at the rear of the magazine) of the vertical cutout. I took a small file to the burrs and all seems well (?). Now to my question. If that small burr did indeed cause the double feed, exactly how does this happen? If not, what is the actual cause? I have been lucky in that in over 45+ years of M16/AR15 use I have never had a double feed event. Now I have had two events in just over 1200 rounds. Because I am not sure of the cause I am slightly uneasy about having this happen at an inopportune moment.

I don't have a scar, but I have an fs2000 which came with the same magazine.

Most likely, the problem is the magazine followers.. On the fnh d&h contract mag I had double feeds and failure to feeds like crazy until I replaced the green follower with a magpul enhanced follower. I actually changed all of my followers out and haven't had a single problem with feeding since.

The rubbing occurs on all of my mags with both my ar15 and my fs2000, it's just something that happens with use.

A double feed in a semi automatic carbine is almost always magazine related.  Keep your mags marked and if they malfunction again toss them.

Both my SCAR Lites magazines came with magpul or magpul type followers. Both my FS's came with crap green followers.

Found that when I tried to use my un-"modified" lancer mags, and pmags, I'd get second round double feeds. Opening up the left side of the mag's  bolt-catch-notch immeiately resolves the issue.  Gotta dremel/file until the bolt catch moves freely without getting hung up on the mag notch.
