hello all,
I have a SBR running a LMT 10.5" upper, and I have been looking at some 11.5" uppers.  My question is:  Can I swap my 10.5" upper out for a 11.5" upper, even though on my form 4 it states that the barrel is a 10.5"

yes,  if it will be a permanent change you should notify the ATF.

Is there any forms I need to fill out to do this?

No forms needed. Tony k suggested a great way to do it. Send two copies of a letter stating that you are the owner of serial # xxxxx, and you want the record updated to show the barrel length as x and the OAL as y. Send a self-addressed stamped envelope and ask them to send back a copy date stamped. This will be your record that they received it. You will get official notice when the record is updated, but that could take up to a couple years. He also suggested sending in a copy of the Form 1 so that they update the correct record.

If you want to permanently change to using the longer barrel, do let ATF know.  But you can have BOTH without any notification to anyone.

I know this topic has been beat to death...  I also read about 32 pages of the "SBR FAQ" and could not find a definitive answer.  I have a carbine length LMT MRP upper with a 10.5" barrel on a registered lower and I would like to purchase multiple barrels all of which would be temporary and would revert back to the original configuration... eventually.  What are the stipulations on this?

Cheers,
Eric

No stipulations.  As long as the lower is registered as a SBR, you can have as many short uppers as you want.  It's a good idea to make sure all your Title I guns have Title I uppers on 'em, though.
