Say I gotta a 9mm upper with a 16inch barrel.  I want it to be an SBR, can I CUT the barrel to my preferred length (4.5") and do some sort of modified tax stamp work or something.  

Anyway to do that legally?

Not really sure what you are asking.. BUT
You fill out a form 1 send it in. If and when it gets approved you can do what ever you want to the barrel length..

OK, I know I can fill out a firm to purchase a short barrel.  I'm wondering if there is a form to fill out that authorizes me to simply cut a barrel to whatever length I want?  Say I have a 16" barrel and want to cut it down to a 4".  Is there a form to fill out making that legal?

When you submit a Form 1 to build an SBR you are registering the receiver of the firearm, not the barrel.  You don't have to fill out a form to purchase a short barrel, you can buy them all day long, just like you don't have to fill out a form to shorten a barrel, however you cannot mate the short barrel to a rifle without first registering the rifle (lower receiver of an AR) as an SBR.

Form 1 is for building your own title ii weapon.  Form 4 is for buying an already built one.  It's $200 either way.

The receiver is the registered part.
