If I have received my silencer and tax stamp from my FFL, do I need to do an Assignment of Property document for it?

I mean, I personally never owned the silencer.  It was transferred from the dealer to the trust, right?

Who is the name on the Form 4? Is it your name or is it blah blah blah Trust?

Yes, you need to fill out an Assignment of Property and list the can on your Schedule A.  This "adds" it to the trust.

When the trust put money down on the counter top, the ownership of said item transfered from store to trust. The trust now can decide the future of the item, if they want it destoyed and removed from the registry or if they want to register it with ATFE.

Yes OP the silencer was transferred to the trust, not to you. You will need to add the silencer to the property assignment sheet. Depending on your state laws, the property sheet might have to be notarized as well for each change.

For Florida I was told the Assignment sheet gets sent in with the Form 4. No notary but it does require 2 witness signatures.

I would be surprised if the BATFE approved your Form 4 with trust without the schedule A identifying property assigned to the trust. If so, yes...Schedule A with any NFA items approved on Form 4. Check state laws for notory of public and/or witness signatures.
