Do i need fingerprint cards for a form 4 and form 1 and a sign off from the chief of police?

Yes, they are both the same as far as their requirements.

A Form 1 is for making and registering an NFA weapon. This would be making your own SBR or suppressor for example.

A Form 4 is for a tax paid transfer of an NFA weapon. This would be buying an SBR or suppressor from a gun shop.

If you have the item registered/transfered to an individual, you need 2 fingerprint cards and 2 copies of the form signed by the chief of police, county sheriff, district attorney, head of the state police, or a judge that handles felonies.

You can also set up a revokable living trust or a corporation and have the item transferred/registered to it, thus no CLEO signoff or print cards.

You can contact a lawyer who can set one up for you, or you can use Quicken Willmaker software. If you go the software route, you have to make sure you know what exactly you have to for YOUR state to make the trust valid. Willmaker justs cranks out a pretty generic trust, and it's up to you to make sure it's legal in your state. You may need to get it notarized, or your state may have other requirements.

Lots of people here use Willmaker and it works fine.
