All right I finally did it, i convinced my wife to let me get a suppressor.  That will hopefully be the hard part of the whole process !  I just have some quick questions to clear up to make sure that I'm doing this right the first time.  So any and all help/feeddback/suggestions is greatly appreciated.

1.  Concerning the Form 1, I should have the Chief of Police sign it first and THEN the Sheriff if the Chief doesn't sign off, or what ?  I live in Montana, so I don't think it will be a problem.
2.  Concerning interstate travel, just want to make sure I'm safe as long as I don't take into a state that doesn't allow them.  Do i need to file any paperwork in advance like I would for a SBR or will my ATF stamp suffice which will ALWAYS be with the suppressor ?
3.  If I were to pass on unexpectedly, and I didn't transfer the suppressor for unforeseen reasons what would my wife have to do as not to get in any trouble ?  i don't have kids yet.
4.  Does the M4-2000 come with the adaptors/special tools to mount in and how long does it take to mount up ?
5.  What is the average time for the stamp to get back.
6.  Could someone "lay-out" the purchase process in a nutshell so this will go smooth, starting with the paperwork/CLEO signoff, purchase, etc ?

Like I said ANY help I can get is appreciated.  Direct all response via email [email protected]/IM me.

The M4-2000 requires you to have a 1/2-28 thread barrel (like AR15s have). You remove the flash suppressor and any washers, degrease the threads, put on 2 drops of the included Rocksett, screw on the included flash hider (also degrease that), and tighten it to 20-30 ft/lbs with a 3/4 inch wrench. You let it set overnight. Then it is ready.

I would bet no wife has ever been in trouble for having their husband's NFA collection. You should tell her and your dealer that if something happens to you, he should sell your stuff and take a 15% commission and give the money to her. Keep her informed of the values of NFA. She can probably sign your name on the paperwork if she has power of attorney. Whatever, the ATF does not go after widows.

What forms do I need and how many of each ?

You need a form-4. Your dealer should fill it out and give it to you to get the Chief to sign it.

Does the form have to be printed back to back? do i have to be present when the dealer fills them out ? the closest dealer is a couple of hours away.  Not a problem if I do have to be there.  Thanks for your responses also.

Yes, it has to be printed two sided. Your dealer should fill it out and mail it to you, and should get it signed, add a check, and mail it to the address the dealer says. No you don't need to be there.

You will also need to include a fingerprint card and a certificate of compliance form.

Your dealer should provide them. You can order them from the ATF if you want.
