I took my m16 out this weekend and on almost every mag 2 or 3 times it would not extract the round and then try to feed a new one.  When I took it apart to clean it the notches in the gas rings where within a 1/8 of a inch from each other.  The bolt is a brand new bolt from bushmaster.  Two weekends ago during the after shooting cleaning session I found a crack through the bolt on one side of the hole where the cam pin is inserted and the other side had a crack a quarter way through.  The gun ran flawlessly that weekend.  Any other things i should look for ?  the upper is a 10.5 inch with a 5.5 inch flash hider ammo is Q3131a.

Try a stronger extractor spring or get a "D-fender" Which is a d shaped piece of rubber which fits around the extractor spring.
