| As an individual, no you do not. The questions on the Responsible Person Questionnaire are on page 2 of the Form 1, so you are answering them there. If an Entity like a trust filed the form 1, they would not answer those questions on the Form 1 and would instead add a Form 23 for each Responsible Person. |
