Hey when y'all receive your e-form, do you fill out the transferee information and sign it again?
Or is the approved pdf good to go as-is? Thanks |

The eForm should have DIGITALLY SIGNED, then your name and the date...in 7, 8 & 9. There's nothing else to do, as long as it has the stamp, examiner signature, and Approved box checked. |
