[ARCHIVED THREAD] - Fingerpring card question

Had some fingerprints done today by the county sheriff for some form 1's i'm going to send in. When the cards were signed by the officer, she put her badge number but did not fill out the address section. Is that ok for sending them in? Also, what should I put in the box for reason fingerprinted....NFA?

sometimes they are filled out and I just sign, sometimes they aren't, and I fill them out.

I put the address of the person fingerprinting me or in your case the PD address. You can do it. As for the reason I put : NFA purchase

Most of mine were sent in with this part blank and they went through without a problem.

I print myself and have never had a problem, Don't sweat the small stuff.
