NFA lost my paperwork after they approved it.... this just sucks........
They advised my to wait until the 18th of this month(30days from approval) before filing the proper paperwork for not receiving my stamp and paperwork back.... |

Lost form 4 just happen to me. My form was approved on the 19th of July and the paper work never showed up at the dealer. I was told to send a letter to West Va requesting a "certified copy" of the approved form (faxed to WV and sent a copy via snail mail). The fax was sent last friday. I got a call from them today (Nichole) and she let me know that the certified copy was mailed yesterday. Should show up in the next couple of days. Good luck. Sam |
| they lost mine as well a few years back. filled out one of those 5320's (?) - watever form it is to transport across state lines and they sent a note back - it was a post-it note hand written that said "no serial # xyz found". i guess i should have sent in a copy of the Form 4 but i thought it was pretty strange. |

FOLLOW UP TO MY EARLIER POST:
Received the "certified copy" of the approved Form 4 yesterday. If course it did not have a stamp but did have a embossed stamp as certification of a true copy. According to West Va. NFA folks has the same weight as the stamped versions. Took possesion of the suppressor yesterday. Sam |
