I need to re-do my finger print cards because my CLEO lost ALL my paperwork including Form 4s, checks, photos, and finger prints. Long story short, re-filling these out as I need to get printed again.
For the Aliases, is it better to leave that blank, or just write 'NONE' in the box? |

Agreed, they changed administration recently so it's a big 'ol mess. Once this is signed off I'm getting it back in person and sending it in myself.
Assuming they don't lose the paperwork...again. Anyways, thanks for the advice on the cards. I'll leave that field blank. |
