Can someone give me the link of the sample form1 done for a trust.  I can't seem to find it.

Okay, I think I found it, but I have a couple of questions on filling out the form 1 for a trust:

Should I use Individual or Corporation/Other Business Entity?
Should the Applicant name be the name of the trust (including "Revocable Living Trust") or just my name?
Anyone know what the overall length will be with a 5.5" Spike's 22 upper?  I am using carbine length handguards so I'm not sure if it's from the handguard or the barrel (which is shorter) and if it's from an extended stock or collapsed.
Last one... when engraving my lower, can I put my initials and then trust or does my name need to be spelled out?  Which should it be:
John Doe Smith Trust  
JDS Trust

I should have all this sent out by next week, so thanks for your help.
