Is there a requirement for the quality of the photo I printed myself on the Form 1?  I used a color inkjet and it's of decent quality (not glossy).  Should I also include a real photo just to be safe?  Thanks.

I print mine off of my half unit's HP Photosmart 8250 which is basically an ink jet. It's a dedicated photo printer but the best quality usually comes from photos printed on photo paper. The ones I sent were printed on the flat single sticky side paper & they were just fine. As long as they can clearly see your mug as recorded in the original photo then you have nothing to worry about.

I have done F1's and inserted the pictures from a bmp into Adobe and printed them on the crappiest old inkjet (The only one I have) and had the pictures come out almost entirely red due to the fact another ink color was low.  The form went through no problem.

Your mileage may vary.  The only draw back I have had is that if I try to make photocopys of the approved form, the pictures come out almost all black...
