[ARCHIVED THREAD] - Form 1 help

I am looking to make my SW5 into a SBR. I have the forms but when I went to complete them I found that I am missing part of the address at manufacture.
My question is how particular is the BATFE when it comes to the manufacture address. This was made by Special Weapons which is now no more.
I have Mesa, AZ and nothing else.
Will this be enough to meet the standard on the form?
Does anyone have an original owner's manual that lists the original business address?

City state abbreviation and USA are all I put down and it was approved

I had 2 approved so far with manufacturer, city & state. You're fine.
